✅ Parental/Guardian consent is required to register any student under 18 years of age.

✅ Please check the consent box at the end of the form to confirm your approval.

✅ All information provided will be used only for educational purposes and kept strictly confidential.

✅ If you have more than one child enrolling, please fill out a separate form for each.

(Highly recommended for students living near the Study Center to benefit from direct teacher interaction, hands-on activities, and valuable peer learning through collaboration and social engagement.)
